Is Benzalkonium Chloride Safe? The Honest Answer
On September 6, 2016, the FDA banned 19 antibacterial ingredients from consumer soaps. Triclosan was the famous one. Triclocarban went with it, along with seventeen others most people had never heard of.
Benzalkonium chloride wasn’t on that list.
It wasn’t cleared either. The agency deferred it, along with two similar ingredients, to give manufacturers time to produce the safety and effectiveness data the FDA said was missing. The deferral ran a year, renewable.
Three years later the FDA finalized a second rule, this one covering hand sanitizers. Twenty-eight of the 31 ingredients under review were rejected. Benzalkonium chloride was deferred again, alongside ethyl and isopropyl alcohol, for the same reason: the studies still weren’t done.
So the honest answer to “is benzalkonium chloride safe” is that the US regulator has been waiting on an answer since 2016 and hasn’t got one. Which is a very different thing from either a clean bill of health or a ban, and it’s not what most articles on this ingredient will tell you.
Where you’re meeting it
Benzalkonium chloride, usually shortened to BAC or BZK, is a quaternary ammonium compound. It kills bacteria by wrecking their cell membranes, and it’s cheap, stable, and doesn’t smell.
You’ll find it in alcohol-free hand sanitizers, where it’s the reason a product can claim to be antibacterial without the drying alcohol. It’s in antibacterial hand soaps and wipes. It’s the preservative in a huge share of prescription eye drops and in some nasal sprays. It’s in first-aid antiseptics, in throat lozenges, and across household disinfectants.
That spread matters, because the risk picture here isn’t about one product. It’s about meeting the same molecule in six places a day.
What the concerns actually are
Not cancer. Not hormones. EWG rates it low for both, and nothing in the literature points that way.
What you get instead is allergy and irritation, and on that front the ratings are blunt. EWG’s Skin Deep database scores benzalkonium chloride a 6 on a ten-point scale. Every other ingredient we’ve broken down in this series scores a 1 or a 2. It also rates allergies and immunotoxicity as high. Its certification program doesn’t restrict it pending paperwork the way it does with milder ingredients. It lists BAC as unacceptable outright: an EWG VERIFIED product can’t contain it at all.
Three specific problems sit behind that.
Contact allergy. BAC is a recognized sensitizer, and because it belongs to the quaternary ammonium family, a reaction to one member can carry over to others. There’s a published case of anaphylaxis in a patient using eye drops preserved with it, reported in Clinical and Experimental Optometry in 2009. That’s rare and it’s the extreme end. Ordinary contact dermatitis is far more common.
The eyes. This is the best-studied problem, and the exposure is bigger than you’d guess: a 2022 review in Eye put BAC in roughly 70% of eye drop products. Long-term use is linked to ocular surface disease, because the molecule damages the cells lining the cornea and conjunctiva, and it accumulates in the cornea and in soft contact lenses. Preservative-free eye drops exist as a whole product category largely because of this one ingredient. If you use drops daily for a chronic condition, that’s a conversation worth having with your ophthalmologist.
The airways. BAC has been used as a preservative in nebulizer solutions, where it can trigger paradoxical bronchospasm in some asthmatics, meaning the treatment tightens the airway it was meant to open. Inhaling it as a spray or aerosol can cause coughing, wheezing, and throat irritation, and people with asthma get it worse.
So should you avoid it?
Here’s where I’d land, and it’s more specific than a yes or no.
In hand sanitizer: switch. Not because BAC is dangerous in a single use, but because there’s a better option that isn’t in regulatory limbo. Plain 60%-plus ethyl alcohol works better against more pathogens, evaporates completely, and leaves nothing sitting on your skin. The alcohol-free version’s whole selling point is comfort, and a good hand cream solves that problem without the tradeoff.
In eye drops: ask. If you’re using drops once for allergies, don’t overthink it. If you’re using them every day for glaucoma or chronic dry eye, preservative-free formulas exist for exactly this reason and your doctor can prescribe them.
If you have asthma or eczema: read labels. These are the two groups where the evidence is clearest, and disinfectant wipes and sprays are the usual exposure.
Everyone else, in a wipe or a disinfectant: fine. Occasional contact with a surface cleaner isn’t the exposure the concerns are built on.
I want to be careful here, because it would be easy to write this piece as a scare and it isn’t one. BAC is not a hidden carcinogen and nobody’s covering anything up. It’s an ingredient with a real allergy record, a genuine problem in eyes and airways, and an unusually long regulatory shrug attached to it. That combination is enough to prefer the alternative when there’s an easy one. It isn’t enough to panic over a hand wipe.
Frequently asked questions
Is benzalkonium chloride safe for skin?
For most people, in occasional contact, yes. What causes trouble is repeat exposure on skin that has already been sensitized. It’s a known contact allergen with a high allergy rating from EWG, and people with eczema or already-irritated skin react more readily. If you’re using an alcohol-free sanitizer many times a day, that’s the pattern most likely to cause trouble.
Is benzalkonium chloride better than alcohol in hand sanitizer?
It’s gentler and it’s less effective. Alcohol at 60% or above is the CDC’s recommendation and works against a broader range of germs, including some that BAC handles poorly. The main argument for BAC is that it doesn’t dry your hands, which a decent moisturizer also solves.
Was benzalkonium chloride banned by the FDA?
No, and that’s the point of confusion. It was deferred from both the 2016 consumer antiseptic wash rule and the 2019 hand sanitizer rule, meaning the FDA asked for more safety and effectiveness data rather than issuing a verdict. It remains legal and widely sold.
Why is benzalkonium chloride in eye drops?
Because it’s a highly effective preservative that keeps a multi-dose bottle sterile once you’ve opened it. The tradeoff is that long-term daily use is linked to ocular surface toxicity, which is why preservative-free single-dose vials exist. For occasional use the preservative is doing a useful job.
